If you were having a panic attack you might feel lightheaded and faint, but you wouldn't have intense shaking in only one place in your body. If you were having a migraine (which I am positive you were not) you would be nauseous, sensitive to light and sound, and you wouldn't describe the pain as "coming and going".

I think you need to talk to another doctor. If you're in the US, I hope you have medical insurance - I personally (especially if this happens again) would ask to be taken to a hospital where they can monitor your symptoms.

If you think the doctor might be right about the panic attack, the next time you have a spell, put your shirt (or a tissue) over your face and breathe very slowly through your hands and shirt for about a minute. This will reduce the amount of oxygen in your bloodstream if you're having a panic attack (which is just an overdose of oxygen!). If you continue to feel faint I'm going to say this is something else.

Eat healthy food regularly, exercise. I hope you figure out what's going on soon!

This could be a migraine which can give you all of the symptoms you describe. You should be taking your headache medication at the first sign of a headache so that you don't have to go through this frequently. For the dizziness, you need to dramatically increase your water intake for the next 2 days and take some tylenol or ibuprofen for any fever that you have. It sounds as if you might have a mild virus which can also set off headaches.

Are the headaches migraines?

Do you have an infection of some sort? You said you have a fever, and are coughing.

Are you anemic?
